    • In the event of an active shooter:

      1. Run.
        • Have an escape plan and route in mind.
        • If you are in charge of children, gather them up and run out of the building if it safe to do so.
        • Do not take anything with you.
        • Keep your hands visible, up in the air.
        • If you are able to escape from the building, run away from the building. Run in a zig-zag pattern and spread out if possible. {shooter_location}
      2. Hide.
        • Hide in an area out of the shooter's view.
        • Turn off the lights.
        • Block the entry to your hiding place and lock the doors if possible.
        • Silence your cell phone and turn of vibrations as well.
        • Do not come out of hiding until police come to you and can safely evacuate you.
      3. Fight.
        • Only fight as a last resort and if your life is in imminent danger.
        • Be confident and aggressive. Attack as a group from different angles. Attempt to distract and incapacitate the shooter by improvising weapons, throwing items, and yelling.
      4. If possible, make an intercom announcement alerting the campus of the danger and its current location.
      5. Call 911 when it is safe to do so.

      Assume the threat is viable until the police give the "All Clear." 

       

      In the event of a shooting:

      1. Call the police: {police}

      3. Clear the area of all other members.
      4. If a staff person needs to administer first aid, make sure to put on latex gloves, contained in the first aid kits. Then, follow the Bloodbourne Pathogen procedures.
      5. Assign staff to exterior doors and secure the building.
      6. Close the building. Get the name and phone number of anyone who insists on leaving before police arrive.
      7. Preserve the crime scene. Rope, tape, or block off the affected area and its surroundings.
      8. Cooperate fully with police.
      9. Find all possible witnesses. Keep them separated. Make sure police know who they are.
      10. Comfort those affected as best you can.
      11. Refer any new media requests or questions to the police and {ceo_name}, {ceo_title}, {ceo_phone}.
      12. After police have released the crime scene, use outside resources to assist in the cleanup of affected areas and the disposal of hazardous materials. Follow blood, body fluid, and chemical disposal procedures.

    • Bomb Threat 
    • In the event of a bomb threat:

      1. Stay calm and ascertain as much information as possible.
      2. Write down the following notes to yourself:
        • Description of voice (male/female, young/old, accent, tone of voice, background noise)
        • Time of call
        • Phone number that call came in on
      3. If you can, ask the caller the following questions and write down the answers:
        • When is the bomb set to go off?
        • Where is the bomb?
        • What does it look like?
      4. Advise everyone not to use their cell phones or other electronic devices as these devices have the capacity to detonate an explosive device.
      5. Evacuate the building according to fire drill exits if the situation is safe.
      6. Call 911.
      7. After evacuation, notify neighboring business (if open for operation) of the bomb threat.
      8. Once emergency services determine the building is safe, regular building operations may resume.

    • Building Emergency 
    • In the event of a building emergency, follow these procedures:

      Loss of Power

      3. Tour the facility to determine where critical or emergency situations exist or may arise. Inoperable fire alarms, technology issues or potential power surges to data processing equipment are possibilities. The elevator will not operate.
      4. Either start the evacuation of the center or move the participants and staff to areas of the building that are lit with natural light.
      5. The use of candles is strictly prohibited and very dangerous. Battery operated lighting or emergency lights should be used. Building emergency lights last approximately 60 minutes.

      In a minor emergency situation not requiring the shutting off of electricity to the total facility, first check to see if power can be shut off at the location of the problem, by a specified breaker switch at the main breaker.

    • Chemical Spills 
    • In the event of a chemical spill, follow these procedures:

      1. When you discover a spill, personally guard it so people cannot walk through the spilled chemical.

      3. Determine the identity of the spilled material and what hazards are present. Read the container label. Refer to the MSDS located at the front desk or in the maintenance area for guidance.
      4. If you experience shortness of breath, dizziness, headache or other abnormal physical conditions as a result of fumes or vapors, immediately leave the area and notify a member of management.
      5. Properly store the spilled chemical and contaminated materials in a labeled container or doubled plastic bag for disposal. Use high-quality bags.
      6. Wash your hands and remove any contaminated clothing after the spilled cleanup is finished.
      7. Do not mix chemicals. Cleaning solutions may react with spilled chemicals.

       

      In the event of a chemical or toxic substance emergency:

      1. Evacuate members and staff from the area as quickly as possible.

      5. Spills should be contained to the extent possible. Leaks should be stopped immediately if possible. Block floor drains. Minimize spreading without endangering personal safety.
      6. Submit a detailed report as soon as possible. Give details of the incident, including names and addresses of all witness. Reports are located in the MSDS folder or you can fill out an incident report.

       

      When disposing of spilled chemicals and contaminated materials:

      1. Do not pour anything on the ground except water.
      2. Do not put hazardous chemicals in the dumpster or wash down the drain.
        • Examples: Flammable products, fertilizers, pesticides, pool chemicals, auto batteries, paint, or roof coatings.

    • Earthquake 
    • In the event of an earthquake:

      People inside the building:

      1. Stay inside, do not go outside or run to another room. If possible, quickly move from glass, hanging objects, bookcases, shelves, and large furniture that can fall. Do not use the elevators as the electricity may go out.
      2. Drop and cover consist of dropping down onto hands and knees to protect people from falling. Then cover the head and neck with arms and hands. Staff will provide assistance into the drop and cover position as needed to people with a disability such as mobility impairments, blind, and/or deaf/hearing impaired.
      3. If possible people will get underneath a sturdy table or desk, in the drop and cover position, and hold on to the structure until the shaking stops. If there is not a desk/table nearby people will get near an interior wall in the drop and cover position.
      4. If in a wheelchair, lock the wheels and cover the head and neck with arms.

       

      If outdoors:

      If outside, stay outside. Move to an open area away from buildings, trees, power lines, and telephone poles.

       

      If in a moving vehicle:

      1. Move the vehicle to the shoulder or curb away from power lines, trees, and underpasses/overpasses.
      2. Stay in the vehicle until the shaking stops, engage the parking brake, and turn on the radio for emergency broadcast information.
      3. If a power line falls on the vehicle stay in the car until emergency services remove the wire.

       

      After an earthquake:

      1. After the earthquake, take caution for aftershocks as objects can fall.
      2. Staff will perform a thorough examination of the premises as it is possible people may be trapped or injured. If anyone is injured and requires emergency services, call 911.
      3. Once the building has been deemed safe, regular building operations may resume.

       

      In the event of any property damage from an earthquake, when it is safe to do so:

    • In the event of a fire, follow these procedures:

      1. Pull the fire alarm (if it is not already sounding) before any other action is taken. This will alert the fire station. If there is no alarm system, call 911.
      2. Staff should make an an evacuation announcement throughout the building:

        "May I have your attention please? This is an emergency. All members and staff must leave the building immediately. {fire_announcement}"

      3. Evacuate the building:
        • Fire exit signs are posted throughout the building.
        • Staff should begin evacuation of the facility using the nearest exit. {fire_evacuation}
        • Help clear all areas per posted fire evacuation plan in each space. Staff should conduct a thorough sweep of the building to ensure everyone has exited the building.
        • If you have a person with mobility issues please direct them to the nearest safe exterior exit and provide assistance as needed.
        • Make sure you inform the fireman in the white helmet or the battalion chief of anyone needing extra assistance or rescued.
        • After evacuation, notify neighboring business (if open for operation) of the fire.

      After the fire department checks the building, the alarm may be reset by the fire department only. Even if you know there is not a fire, the building must be evacuated if the alarm has been sounded.

       

      When it is safe to do so:

    • In the event of a hostage situation:

      1. Stay calm. Be cooperative. Do not threaten or intimidate.
        • Remember that lives are your top concern. Before you say or do anything, think of the consequences to your members and staff. Their safety, and yours, comes first.
      2. Keep lines of communication open with the hostage-taker. Because they are in temporary control of the situation, you must communicate.
      3. If anyone in the building needs medical attention, encourage the hostage-taker to allow them to be treated.
      4. Call the police only if it will not put you or anyone in the building at risk.
      5. Make mental notes about the hostage-takers.
        • Age, skin color, hair color, eye color, voice characteristics (accents, high/low pitch, raspiness, etc), color and type of clothing
      6. If safe, evacuate the building.

       

      When the hostage situation is ended:

      1. If anyone is injured, get medical help immediately.
      2. Make sure police have been notified.
      3. Find witness and write down their names, addresses, and phone numbers. Try to keep witnesses on the scene. Provide your notes, with witnesses' names, to the police, if requested, and to your supervisor.

       

      When it is safe to do so:

    • Injury 
    • In the event a member is injured while in our building or on our property:

      1. Listen carefully to the member's comments. Have a "we care" attitude.
      2. Check the scene. If the injury appears to be serious or the member requests it, call 911. Notify the Front Desk and clear area of other members.
        • If 911 is called, make sure to inform your supervisor and {risk_manager_name}, {risk_manager_title}, once the victim has been transported.
      3. If the injured person is a minor, call a parent or legal guardian immediately. If parent or legal guardian is present, obtain consent before providing care.
      4. Render first aid immediate if certified. If CPR is required, begin immediately and continue until the AED arrives, is turned, pads are applied, and it is analyzing rhythm or the paramedics take over.
        • Do not attempt to move the person unless area is unsafe.
        • Always use a protective barrier (gloves, CPR mask, etc), located in the first aid kids.
      5. Fill out an incident report.
        • Do not give a copy of the incident report to the injured party. The incident report is for YMCA and its insurance company's use only. 
      6. Show compassion (remember: we care) but do not commit to paying medical bills or admit fault until an investigation is completed. If the member has questions, refer them to {executive_director_name}, {executive_director_title}, {executive_director_phone}.

       

      When it is safe to do so:

    • In the event you are contacted by the news media requesting information or comments on any matters involving the YMCA of Greater Louisville, follow these procedures:

      1. Do not make any statements regarding the incident. Do not respond with “No Comment.” A response to the media should be: "{ceo_name} has been designated to speak on behalf of the YMCA of Greater Louisville. {ceo_name} is the {ceo_title} and can be reached at {ceo_phone}. {ceo_name} has all of the current information related to the incident to more effectively respond to your questions.”
      2. Take down the reporter’s name, station or newspaper and telephone number to assist {ceoName} and spokesperson of the Y. Be courteous and helpful to the media if they need assistance in contacting this person or their office.

    • Missing Child or Kidnapping 
    • In the event a parent or legal guardian tells you their child is missing, follow these procedures:

      1. Obtain a detailed description of the child
        • Name, age, hair color, approximate height/weight, what the child was wearing (color and type of clothing, especially the shoes)
      2. Take the parent/legal guardian to the Front Desk.
      3. Staff must be posted at all exterior doors to make sure the child does not leave the building without a parent or legal guardian. If there are not enough staff available to place at all doors, the main street door and the exit door near the Kids' Club or preschool room should be prioritized.
      4. Lifeguards should clear pools immediately. All other staff will search the building and grounds for the child. 
      5. After a complete search, if the child is not found, consult with the parent/legal guardian about calling the police.
      6. Search for the child will continue until the child is found or police arrive.
      7. If the child is found and is not with the parent/legal guardian, try to keep the person in the building. Do not use force to stop the person from exiting the building. Call the police: {police}.

       

      When it is safe to do so:

    • Reasonable Suspicion of Alcohol or Drug Use 
    • In the event there is reasonable suspicion that an individual is under the influence of alcohol or drugs:

      1. Observe the individual carefully for specific signs.
        • For alcohol, this may include smell of alcohol, slurred speech, unsteady balance, aggressive or unusual behavior, or bloodshot eyes.
        • For drugs, look for dilated or constricted pupils, paranoia, confusion, hallucinations, excessive sweating or drowsiness, sudden mood changes, or visible needle marks or nasal residue.
      2. If reasonable suspicion exists, remove the individual from any safety-sensitive duties immediately. Notify your Executive Director and the individual's employer (if not a YMCA employee) without delay. 
      3. Document exactly what you see in real time, including who was involved, what was observed, when and where the behavior occurred, and how it was observed.
        • Use clear, objective, descriptive language and avoid vague statements such as “seemed off” or “looked high.”
      4. Follow YMCA escalation protocols and contact the police if applicable.

       

      After the individual has been removed and the situation has ended:

      1. Ensure that anyone who is injured receives medical attention immediately.
      2. Confirm that the police have been notified.
      3. Locate witnesses and record their names, addresses, and phone numbers.
        1. Try to keep witnesses on the scene until police arrive.
      4. Provide your notes, along with the witnesses’ information, to the police if requested, and to your supervisor.
      5. Store all written records securely and maintain confidentiality at all times.

    • Robbery 
    • In the event of a robbery, follow these procedures:

      1. Stay calm. Obey the robber's instructions. Do not attempt to stop or apprehend the robber.
      2. Remember the robber. Try to calmly note the physical description and dress of the robber (including physical characteristics). Do not be obvious, only do this if it won’t put you in jeopardy. Be observant, but do not stare at the robber.
      3. Remember the getaway car. Note the description of the escape vehicle, if possible, and the direction of travel.

      Under no circumstances is anyone to follow the criminal out of the building or write down information in front of them. This may antagonize the criminal and put you, your members and co-workers in severe danger.

       

      The director or building supervisor should:

      1. Call the police: {police}
      2. Separate witnesses. Have them write down what happened, what was said, descriptions, etc.
      3. Make the witnesses available to the police.
      4. Refer any news media inquiries to the police and {ceo_name}, {ceo_title}, {ceo_phone}.
      5. Preserve the crime scene until the police arrive. Do not touch any objects with which the robber had contact.

    • In the event of lightning/thunder:

      Outdoor Programs

      Any outdoor programs on campus should be notified of severe weather. Staff and participants should move indoors. Outdoor activities must be paused for 20 minutes any time lightning is seen or thunder is heard. 20-minute cycle resets every time there lightning is seen or thunder is heard.

       

      Pools

      1. Make sure lifeguard(s) on duty are aware of the conditions.
      2. Lifeguard(s) clears pool with a long whistle blast.
      3. Lifeguard(s)/instructor(s) clears pool deck and directs members to locker room facilities.
      4. Lifeguard(s) posts "Pool Closed" and "No Lifeguard on Duty" signs and locks doors.
      5. Lifeguards wait in lobby until severe weather clears. Once cleared, take down signs and notify Front Desk that pool is open.

      Pool must be cleared for 20 minutes any time lightning is seen or thunder is heard. 20-minute cycle resets every time there lightning is seen or thunder is heard.

       

       

      In the event of a tornado alert:

      1. A tornado watch means there is a potential for tornadoes to develop. Keep an eye on the weather. Listen to the TV, radio, or check the Internet for special bulletins and updated information.
      2. A tornado warning means a tornado has been sighted. A director should make the following announcement, in a calm voice, in all areas of the building:

        "May I have your attention, please. The National Weather Service has issued a tornado warning. Please calmly move to the locker rooms or other designated area and remain there until the "All Clear" is given. Our employees will guide you to the proper areas."

      3. Staff will assist with escorting all those within the facility to a safe area in a quick and safe manner.
        • In the event YMCA program participants/staff are on an outing during a severe weather event, staff will ensure all participants seek shelter at that location. Staff will perform a headcount to ensure the presence of all program participants and fellow staff members.
        • If in a vehicle during a tornado or tornado warning seek shelter immediately in a building i.e. a store, restaurant, etc. During a tornado, if stuck in heavy traffic and there is nowhere to go, duck and cover in a ditch. Ditches should be used only as a last resort as exposure to flying debris, rain, hail, wind, and lighting exists. If forced to stay in the car during a tornado, everyone is to keep on their seat belt and cover their heads below the windshield/windows to protect from glass. Do not go under an underpass.
      4. Know your safe spots. They are found in interior rooms or interior halls. Sit on the floor with your head facing the wall. Use your arms to protect your head and neck.
      5. Know your danger spots - near exterior walls, windows, glass, mirrors, unsecured or hanging objects, and around chemicals.
      6. Do not allow anyone to leave the building until they have been warned of the condition. If an adult (over age 18) refuses to go to the designated area and insists on leaving the building, that is their choice. However, they may not remain in a non-designated area of the building.

       

      In the event of a flash flood or rising water:

      1. Clear members from the danger area.

       

      In the event of any property damage from storms, flooding, or other severe weather, when it is safe to do so:

    • Vehicle Safety 
    • Prerequisites to operating a YMCA-owned vehicle:

      • Drivers of YMCA-owned vehicles must be YMCA of Greater Louisville employees.
      • Drivers must possess a valid driver’s license and be at least 21 years of age.
      • Drivers are subject to a driving assessment and/or additional transportation procedure trainings.
      • Drivers are subject to background checks that include driving records.

      While operating a YMCA-owned vehicle:

      • The number of passengers must not exceed the original design of the vehicle.
      • Seat belts must be operable and worn by everyone.
      • Driver must be in possession of a mobile phone.
      • Prior to departing, the following procedures should be followed:
        1. Confirm tires are properly inflated.
        2. Complete a “circle of safety” check around the vehicle.
        3. Adjust rear-view mirrors.
        4. Fasten seat belts.
        5. Check gas levels.
      • Speed limits and parking notices must be observed at all times. Speeding, parking, and other citations are the responsibility of the person to whom the citation was issued. If a violation is issued to a staff member while operating a YMCA vehicle, the violation should be reported to the appropriate supervisor.

      In the event of an accident involving a YMCA-owned vehicle:

      1. Survey the scene for injuries to passengers. Seek emergency aid for any person showing or complaining of injuries.
      2. Contact police and submit an accident report.
      3. License numbers and vehicle registration numbers should be obtained by both parties involved in the accident.
      4. Parents and/or legal guardians of passengers must be informed of the accident immediately – as soon as possible on the same day accident occurred.
      5. Fill out an incident report.

      In the event of a roadside emergency involving a YMCA-owned vehicle, such as a flat tire or disabled vehicle:

      1. Survey the scene and ensure all passengers are moved to safe area.
      2. If you know your branch maintains a AAA membership, locate the YMCA AAA card within the vehicle and contact AAA.       4. If dangerous persons are outside the building: secure the building, lock all exterior doors and limit any movement or activities near windows or glass doors.
      5. Fill out an incident report.
